The Royal College of General Practitioners are seeking to recruit champions to be part of a network working with the RCGP’s high-profile Collaborative Care and Support Planning (CC&SP) programme, and NHS England’s Long Term conditions team, to support general practice teams and CCGs to implement and understand care and support planning and person centred care.
Champions will be recruited from a range of professionals including social workers, nurses, doctors and other clinical specialists. The network will be fundamental to making this a reality on the ground, driving the culture shift towards this new way of working across the UK; working within one or more of the following categories:
- Regional champions –these champions will represent a geographical area in the UK.
- Process champions– these champions will work at a national level. They will have particular skills or knowledge around one area of the CC&SP process, for example, discussion, or in the system requirements such as IT systems, data and measurement. They will provide guidance to the RCGP and to the rest of the network on that their specialist area, as well as offering support to healthcare professionals on the ground.
- Clinical speciality champions – thesechampions will work at a national level. They will be clinical experts in an area of medicine that would benefit from CC&SP or person centred care, for example, mental health. They will offer clinical expertise to the RCGP and the network and direct support to practices where necessary. They will work to align the CC&SP agenda with that of their medical speciality to maximise potential benefits for patients.
